转化证据按Tier I(对照人体AR证据)至Tier IV(细胞、组学或计算证据)分级,同时单独评估机制置信度。
Allergic Rhinitis (AR) is an allergen-driven, IgE-mediated inflammatory disease of the nasal mucosa. Established therapies are effective for many patients, but incomplete symptom control, suboptimal adherence, and treatment preference sustain interest in new locally acting agents. Plant-derived alkaloids are frequently proposed as anti-allergic candidates; however, the literature often conflates natural compounds with semisynthetic derivatives and synthetic analogues and gives similar weight to computational predictions, cell assays, animal models, and clinical findings. A structured critical review of chemically defined alkaloids investigated in AR was conducted. Translational evidence was graded from Tier I (controlled human AR evidence) to Tier IV (cellular, omics or computational evidence), while mechanistic confidence was assessed separately. Nine canonical plant alkaloids had Tier II evidence from direct AR or combined allergic rhinitis and asthma syndrome models with nasal outcomes: berberine, corynoline, piperine, sinomenine, higenamine, dictamnine, ellipticine, warifteine and methylwarifteine. Reported effects included fewer nasal symptoms, reduced mast-cell activation, lower IgE and type 2 cytokine responses, and less eosinophilic inflammation. Warifteine and methylwarifteine constitute the most internally coherent preclinical series; most other candidates depend on a single short-duration, ovalbuminbased study. Randomisation, blinded outcome assessment, clinically relevant comparators, nasal pharmacokinetics and direct target-engagement experiments were uncommon. Safety is compound-specific and materially changes translational rank. Relevant concerns include drug-drug interactions, non-IgE-mediated mast cell activation, systemic adrenergic effects, reactive metabolites, and genotoxicity. Intranasal ipratropium provides controlled human evidence for watery rhinorrhoea, but it is a purpose-designed synthetic atropine derivative and cannot validate plant alkaloids as a therapeutic class. The available evidence therefore supports the use of selected alkaloids as lead scaffolds and mechanistic probes, but not as established AR treatments. Progress requires authenticated chemistry, parent- and metabolite-aware nasal exposure, causal studies in human nasal systems, active-comparator efficacy and safety margins appropriate for long-term treatment of a usually non-life-threatening disease.
