OBJECTIVE(S): Recently, the role of vascular risk factors in developing Meniere's disease (MD) has been proposed. Brain MRI has been obtained from MD cases to rule out tumor lesions in CPA and Labyrinth. In this study, we investigated the frequency of vascular risk factors and white matter lesions (WMLs) in brain MRIs of MD patients and the control group.
METHODS: In a case-control study, 30 definite unilateral MD cases and 30 individuals as the control group (aged 18-65; none had migraine) were enrolled. Their demographic and clinical information was collected through questionnaires, and brain MRI was performed and interpreted in both groups. The extent of WMLs was assessed using the Fazekas criteria.
RESULTS: The patient and control groups showed no significant differences in terms of mean age, sex (p = 0.08 and p = 0.3, respectively), and any vascular risk factors. 60% of MD patients and 23.33% of individuals in the control group had WMLs on MRI (p = 0.004). Indeed, MD was associated with a nearly 5-fold increase in WMLs in MRI. The difference between the two groups in periventricular white matter (PVWM) lesions was more significant (P = 0.032), compared with deep white matter (DWM) lesions (P = 0.052).
CONCLUSION: Despite no association between vascular risk factors and MD in this study, WMLs were nearly 5-fold more prevalent in the brain MRI of MD patients, particularly PVWM-type lesions. It seems that aside from detecting tumoral lesions and structural abnormalities, MRI has more findings in MD subjects.
