A 14-year-old male with repaired cleft lip and palate presented with unilateral foul-smelling nasal discharge, obstruction, and facial pain. CT identified an intranasal ectopic tooth with chronic sinusitis. Surgical removal using a Caldwell Luc approach resulted in symptom resolution. Early diagnosis and imaging are crucial for effective management and preventing complications.
