Paraesophageal hernia repair is an effective operation for patients with symptomatic hernias. Proper patient selection to rule out concomitant comorbidities and appropriate workup is paramount prior to the operation. Surgeon experience with the operation is important to avoid intra-operative pitfalls and injuries to upper abdominal viscera. Common intra-operative pitfalls are related to crural tension and managing a short esophagus. Despite being highly effective for symptom control, many patients experience some degree of dysphagia, and long-term radiologic recurrence remains high.
