ObjectiveTo evaluate whether a smartphone-based gaze stabilization test (mVOR) detects unilateral vestibular dysfunction (UVD) compared with computerized gaze stabilization testing.MethodsAdults with UVD (n = 18) and controls (n = 17) performed horizontal GST using mVOR and a commercially available computerized system. Outcomes were GST threshold (°/s) and lesioned vs non-lesioned differences.ResultsmVOR detected lower lesioned-side thresholds than non-lesioned (Δ -16.3°/s; p = 0.006) and lower lesioned-side values vs controls (100.9 ± 34.0 vs 142.9 ± 12.5°/s; p < 0.001). Computerized GST showed similar patterns (Δ -8.5°/s; p = 0.042; patients 84.0 ± 23.1 vs controls 111.2 ± 22.6°/s; p = 0.001).ConclusionsmVOR differentiated UVD from controls and lesioned from non-lesioned performance, with patterns comparable to those of computerized GST, supporting the feasibility of a scalable assessment.
