钇-90微球选择性内放射治疗（Y90-SIRT）是肝细胞癌和肝转移瘤常用的局部介入治疗方法。随着其应用日益增多，肝外并发症逐渐被认识。我们报告一例47岁肝细胞癌（HCC）患者，在接受Y90-SIRT治疗后逐渐出现远端食管溃疡并继发良性瘢痕性狭窄，临床推测为放射性所致。同时使用的仑伐替尼可能损害黏膜修复功能。这一罕见后遗症强调，尤其对于接受联合靶向治疗的患者，应进行严格的术前血管评估、长期监测和及时干预。

Yttrium-90 microsphere selective internal radiation therapy (Y90-SIRT) is a common local interventional treatment for hepatocellular Carcinoma and liver metastases. With the increasing use, extrahepatic complications have been gradually recognized. We report a case of a 47-year-old patient with hepatocellular carcinoma (HCC) who gradually developed distal esophageal ulcers and subsequent benign cicatricial stricture, clinically presumed to be radiation-induced, following Y90-SIRT treatment. Concurrent lenvatinib may impair mucosal repair function. This rare sequela emphasizes rigorous preoperative vascular evaluation, long-term monitoring and timely intervention, particularly for patients undergoing combined targeted treatment.
