BACKGROUND: People with diabetes have an increased risk of falls, potentially related to impaired postural control, peripheral neuropathy, and vestibular dysfunction. This secondary fall-focused analysis examined whether video head impulse test (vHIT) and computerized dynamic posturography (CDP) measures were associated with previous self-reported falls in the DIAFALL cohort.
METHOD: Fall data were available for 108 participants aged 40-80 years: 11 controls, 49 with type 1 diabetes (T1D), and 48 with type 2 diabetes (T2D). Previously collected vHIT and CDP measures, including Sensory Organisation Test (SOT) and Motor Control Test outcomes, were analysed in relation to falls within the previous 12 months. Logistic regression examined associations with falls, and secondary Spearman analyses explored selected clinical markers.
RESULTS: vHIT and CDP measures did not consistently identify participants with previous falls. Lower SOT4 scores were associated with falls in unadjusted analyses in the total cohort (p = 0.02) and T1D subgroup (p = 0.04), but not after adjustment (p = 0.54 and p = 0.11, respectively). In T1D, lower left anterior vHIT gain was associated with falls, but this finding was based on few events and unsupported by other canal measures.
CONCLUSION: vHIT and CDP did not robustly identify previous self-reported falls. Findings were exploratory and require confirmation in prospective studies.
