PURPOSE: To validate eight commonly used International Classification of Diseases, 10th Revision (ICD-10) diagnosis codes for childhood sensorineural hearing loss registered in the Danish National Patient Registry.
METHODS: We conducted a registry-based validation study in the Central Denmark Region. A random sample of 500 children younger than 18 years with at least one predefined ICD-10 hearing loss code registered between February 2019 and December 2021 was selected for medical record review. Codes were validated against documentation from ear-nose-throat and audiology services, including descriptions of audiological test results. Positive predictive values (PPVs) with 95% confidence intervals were estimated for each ICD-10 code. We also assessed whether each code identified children with sensorineural hearing loss (SNHL), irrespective of exact subtype or laterality.
RESULTS: Of the 500 sampled children, 465 were eligible for validation of ICD-10 codes and 457 for classification of SNHL. PPVs ranged from 0.92 to 0.99 when primary and secondary diagnoses were combined. Lower PPVs were observed for codes representing mixed hearing loss. PPVs for classifying coded cases as sensorineural hearing loss exceeded 0.90 for most codes. The unspecified hearing loss code showed reduced performance when used as the sole registered diagnosis.
CONCLUSION: ICD-10 diagnosis codes for childhood sensorineural hearing loss in the Danish National Patient Registry demonstrate high validity and are suitable for registry-based research. Codes for mixed and unspecified hearing loss should be used with caution in studies requiring precise phenotypic classification.
