The relationship between obstructive sleep apnea (OSA) and hypertension has been well established. The Advanced Lung Cancer Inflammation Index (ALI), which integrates markers of inflammation and nutritional status, has been associated with hypertension in OSA patients. This study aimed to explore the association between ALI and hypertension among individuals with OSA. Data from 12,049 participants across four National Health and Nutrition Examination Survey (NHANES) cycles were analyzed. OSA was identified using a questionnaire-based sleep-related information. Logistic regression models adjusted for confounders and restricted cubic spline (RCS) analyses were conducted to evaluate the associations and potential nonlinear relationships. Logistic regression analysis showed that each unit increase in Log ALI was associated with higher odds of hypertension (OR = 1.44, 95% CI: 1.25-1.66, P < .001). Quartile analysis demonstrated that participants in the fourth ALI quartile had higher odds of hypertension than those in the lowest quartile (OR = 1.70, 95% CI: 1.40-2.07, P < .001). RCS analysis supported the nonlinear association between ALI and hypertension. This study suggests a significant association between ALI and hypertension in individuals with OSA, highlighting the potential relevance of inflammation and nutritional status in hypertension.
